The fruit of the saw palmetto tree does not relieve symptoms of an enlarged prostate, even when men take the herbal supplement in very high doses, a new study shows.

The research is published Sept. 28, 2011, in the Journal of the American Medical Association.

Many older U.S. men take saw palmetto extract in an attempt to reduce bothersome symptoms of a swollen prostate, including frequent urination and a sense of urgency. Its use in Europe is even more widespread because doctors often recommend saw palmetto over more traditional drug treatments.

Results of the new study may settle an ongoing debate over the effectiveness of saw palmetto for a condition known as benign prostatic hyperplasia (BPH). Earlier studies of the supplement have produced conflicting results, and none have evaluated the benefits of saw palmetto in high doses.

In the current study, however, men took up to three times the standard dose of saw palmetto.

"Now we know that even very high doses of saw palmetto make absolutely no difference," says co-author Gerald Andriole, MD, the Robert K. Royce Distinguished Professor and chief of urologic surgery at Washington University School of Medicine in St. Louis. "Men should not spend their money on this herbal supplement as a way to reduce symptoms of enlarged prostate because it clearly does not work any better than a sugar pill."

The multi-center study, led by Michael Barry, MD, at Massachusetts General Hospital in Boston, involved more than 300 men ages 45 and older who had moderate symptoms suggestive of an enlarged prostate, such as frequent urination, difficulty emptying their bladders and a weak urine stream. The men were randomly selected to receive a daily dose of saw palmetto extract, beginning at 320 milligrams, or an identical-looking placebo pill with the same distinctive smell and taste.

After 24 weeks, the saw palmetto dosage was increased to 640 milligrams a day, and after another 24 weeks, to 960 milligrams a day ? triple the standard dose. In all, men took saw palmetto or a placebo for nearly 17 months. Neither the physicians nor the patients knew who was taking what regimen until the end of the study.

The researchers found that among men who took saw palmetto, prostate problems improved slightly but not more than in men taking a placebo.

"We commonly see this in clinical trials," Andriole explains. "Patients often report an improvement in symptoms because they are taking something, even if it is a placebo. But in this study, there was no benefit to taking saw palmetto over the placebo."

The researchers found that saw palmetto had no greater effect than the placebo on BPH symptoms as well as other conditions related to an enlarged prostate such as waking at night to urinate, PSA level and bladder control.

About half of all men over age 50 have BPH, which becomes more common as men age. If urination becomes difficult or painful, Andriole advises men to visit their physician. Several approved medications, such as alpha-blockers and 5-alpha reductase inhibitors, are available to successfully treat the condition.

There?s a lot more to the mini-med plans than the controversy over the waivers ? which is how most people know about them. But that controversy has been a constant source of political headaches for the Democrats.

Republicans insist the waivers are political gifts from the White House to its friends ? especially those in labor. The administration says that?s not true and that they?re a necessary stopgap until 2014, when the rest of the health care reform law kicks in.

Now, however, the Department of Health and Human Services is winding down the waivers, which help the plans keep lower benefit limits than they?re supposed to have under the law. The deadline for applying to the waiver program expired last week, but the waivers themselves can last another two years.

To hear critics of the mini-med plans tell it, the coverage barely counts as insurance. ?It gives people a false sense of security,? Sen. Jay Rockefeller (D-W.Va.) said during a December 2010 commerce committee hearing on mini-meds. ?It lets [people] think they have health insurance when they really don?t. By the time they realize they don?t have real health insurance, it?s too late.?

But HHS reasoned that even the worst insurance is better than no insurance. So like it or not, mini-meds are here to stay until at least 2014. And that presents policy challenges ? some immediate and others longer term.

What about the next two years?

These obscure health plans ? also called limited-benefit plans ? were thrust into the spotlight last year when The Wall Street Journal reported McDonald?s Corp. had considered dropping coverage for its nearly 30,000 hourly restaurant workers because it wouldn?t be able to meet the health reform requirement known as the medical loss ratio, which limits health plans? administrative costs.

Both McDonald?s and HHS denied the report, but the issue lingers because the Obama administration last December granted the mini-med plans a one-year exemption from the MLR requirement to spend at least 80 percent of premium dollars on health services. The skimpy plans? low benefit payouts and relatively high administrative costs make the MLR threshold a pretty big challenge.

Little is known about how implementing the MLR would affect mini-meds, so HHS has been collecting financial data on the plans and is scheduled to issue updated guidance on MLR requirements this year.

?It?s a gray market,? said Sabrina Corlette, a research professor at Georgetown University Health Policy Institute. ?We need to know a lot more about it.?

"This might be a result of a link to the heavy snow year, with heavier and longer runoffs," John Chang, chairman of the California Mountain Rescue Association, said via email. The addition of a mild summer may have "led to more people being caught off guard."

Senator not always critical of energy loan program (AP)

WASHINGTON ? Republican Sen. David Vitter calls the Obama administration "reckless" for awarding billions of dollars in taxpayer subsidies for renewable energy projects, including a $528 million loan to a now-bankrupt California solar panel maker. But Vitter was not always so critical of the loan program.

Documents obtained by The Associated Press show the Louisiana Republican wrote to the Energy Department at least seven times since 2009 seeking money for projects that would benefit his home state.

One of the projects backed by Vitter ? for a company that makes activated carbon to reduce pollution at coal-fired power plants ? has received preliminary approval for a $245 million loan guarantee.

In April 2009, Vitter urged Energy Secretary Steven Chu to approve a loan for Red River Environmental Products, saying the Coushatta, La., company could help meet a growing demand for products that help power plants comply with stricter federal regulation of mercury emissions.

"I understand the importance of accessing the domestic energy resources we have, like coal, in an environmentally conscious manner," wrote Vitter, who also backed projects for nuclear power, renewable diesel fuel and a company that makes fuel-efficient cars.

Vitter and other Republicans have pounced on the bankruptcy of Fremont, Calif.-based Solyndra Inc., saying the White House rushed to approve a loan guarantee to the politically connected company without adequate oversight

Vitter said in a statement Monday that he has "always pushed for an `all of the above' energy policy which certainly includes renewables," but added: "In the age of Solyndra, we need full transparency and accountability."

Rep. Cliff Stearns, R-Fla., who chairs a subcommittee that is investigating the Solyndra deal, also has supported projects that promote green jobs.

Stearns, who heads the House Energy and Commerce subcommittee on oversight and investigations, endorsed a battery manufacturing plant in Jacksonville. The Saft America Inc. plant makes lithium-ion battery cells for military hybrid vehicles and solar and wind energy storage. The plant received a $95.5 million grant from the Energy Department through the stimulus law.

Stearns and other lawmakers from Florida also backed a Florida company's bid to win a loan guarantee for biofuel refinery plant in central Florida. New Planet BioEnergy LLC received $50 million from the Energy Department and a $75 million loan guarantee from the Agriculture Department.

"I am honored to join in welcoming Saft's Li-ion battery manufacturing facility," Stearns said in a March 2010 press release announcing the battery plant's groundbreaking. "I recognize the contributions of these advanced rechargeable batteries in meeting our energy needs."

A spokesman for Stearns said the congressman did not attend the groundbreaking last year, but merely provided a statement in support. Stearns also did not attend a ceremony last week when the plant formally opened with 100 employees, spokesman Paul Flusche said. Several Florida members of Congress attended the opening event.

Stearns said in a statement Monday that he supports clean and renewable energy and the jobs they produce, nationally and in Florida, "as long as taxpayer dollars are not put at risk."

In Saft's case, the company received a federal grant, not a taxpayer-guaranteed loan, Stearns said. "I had no role in Saft securing that grant; there is no indication that Saft, a French company, is financially troubled; and it has not been raided by the FBI," as Solyndra was.

In response to the Solyndra Inc. debacle, Vitter has co-sponsored a bill that would require an inspector general investigation into any company that receives federal money for renewable energy and then goes bankrupt.

"We can't afford any more crony capitalism where the federal government picks winners and losers and then leaves taxpayers on the hook when everything falls apart," Vitter said in a news release announcing the bill, which is co-sponsored by Sen. Ron Johnson, R-Wis.

Vitter's bill would also require that federal agencies conduct a full audit of any renewable energy projects that have received taxpayer money since 2009.

The audits would examine how many jobs were created and what the company's profits are. Any company that declares bankruptcy or fails to meet the objectives required by the government would be subject to an inspector general investigation.

Solyndra filed for Chapter 11 bankruptcy protection earlier this month and laid off its 1,100 employees. The Silicon Valley company was the first renewable-energy company to receive a loan guarantee under a stimulus-law program to encourage green energy and was frequently touted by the Obama administration as a model.

The company's implosion and revelations that the administration hurried Office of Management and Budget officials to finish their review of the loan in time for a September 2009 groundbreaking have become an embarrassment for President Barack Obama.

Elite controllers block integration of HIV DNA into host genome

ScienceDaily (Sep. 19, 2011) ? Alone among those infected with HIV-1, so-called elite controllers spontaneously maintain undetectable levels of viral replication even absent the benefit of anti-retroviral therapy. Now Mathias Lichterfeld of the Massachusetts General Hospital, Boston, and Xu Yu of the Ragon Institute show that in elite controllers, integration of HIV-1 DNA into the host chromosomes of CD4 T cells -- the main target cells of HIV-1 -- is markedly reduced in comparison to those whose infection has run a more normal course. "[Elite controllers] behave like people who get effective antiretroviral treatment, despite the fact that they don't," says Lichterfeld.

In the study, the researchers removed CD4 T cells from elite controllers, from random HIV-1 negative persons, and from HIV-1 infected persons with progressive disease, and infected those cells with HIV-1 in the laboratory. While HIV-1 successfully integrated into both reference populations' CD4 T cells far more effectively than into those of elite controllers, the researchers found higher levels of unintegrated, extrachromosomal HIV-1 DNA floating around in the elite controllers' CD4 T cells.

"Overall, this suggests that the process of chromosomal integration of HIV-1 is somehow inhibited in elite controllers," says Lichterfeld. Now poorly understood, the mechanism likely involves a synergistic interplay between multiple innate and adaptive immune defenses, he says.

"We think that these subjects can really teach us a lot about how immune-mediated control can work under real-life circumstances," says Lichterfeld. "If we were to understand in detail what's going on in these patients, we might be able to develop some sort of intervention that could protect people against HIV-1."

This report is consistent with a paper published earlier this year which showed that elite controllers have low levels of chromosomally integrated HIV-1 DNA, and higher levels of extrachromosomal, 2-LTR circular HIV DNA, as compared to patients on highly active anti-retroviral therapy (HAART) (PLoS Pathog. 7:e1001300).

The research is published in the September 2011 issue of the Journal of Virology.

French news reports said Sarkozy was accompanied by dozens of French riot police, an unusual move. While the former rebels have control over most of the country, Gadhafi is still on the run and his loyalists are still holding out in three main strongholds in central and southern Libya.

Despite the continued instability, Libya's interim government, led by Mustafa Abdul-Jalil, has been trying to assert control and establish international legitimacy. The NTC has asked the United Nations for assistance as it struggles to establish a government.

The Security Council is considering a new resolution that would establish a U.N. mission in Libya, unfreeze assets of two major oil companies and lift a ban on flights by Libyan aircraft, according to a copy obtained Wednesday by The Associated Press.

Will Gov. Perry Let Racism-Tainted Sentencing Stand? (ContributorNetwork)

COMMENTARY | The guilt of Duane Edward Buck is not in question in the case of the deaths of two Houston residents in 1995. But the sentence to be imposed, death by legal injection, is coming under fire, in part because of testimony during the sentencing hearing and partly because Gov. Rick Perry is the current front GOP frontrunner in the race for the presidency.

During the sentencing hearing, Walter Quijano stated Buck was more likely to reoffend than others because he is black. According to the Daily Caller, the same doctor testified at six other cases, and the defendants in each case were given retrials after then-Attorney General John Cornyn admitted the state had erred in relying on race-based testimony. So far, Buck's plea for a retrial has been ignored and Buck is slated to be the 235th person executed during Perry's tenure.

It's a tough sell to say the Texas execution law is racially biased. Forty-one percent of those killed by the state are black, in contrast to 12 percent representation in the population. Forty-two percent killed are white in contrast with a 45 percent representation in the population. Sixteen percent of Hispanics on death row in the state are executed; Hispanics make up 37 percent of the overall population.

So the case has to be looked at on its own merit. No matter his race, when a psychologist testifies in Texas that a criminal is more likely to reoffend on the basis that he's black, there can be no doubt the case of that particular criminal is tainted by racism, and the sentencing process should be nullified and revisited by a fresh jury and untainted testimony.

Further complicating the case is the stance of a third victim, who lived through Buck's attack. "This execution would only add to my pain, and it wouldn't give me closure," said Phyllis Taylor, according to the Texas Tribune. "I would ask that Duane Buck's life be spared. I feel that he deserves a fair trial."

Perry recently said he doesn't lose sleep over people killed by the death penalty in the state. But perhaps it's time for the tough-talking governor to stop being candidate Perry for the sake of doing his job as governor. When in doubt of impropriety, clemency is always in order.
